Fees and Policies
Out of Network
Dona Wellness Clinic is considered “out of network” and does not accept any insurance plans/payments from insurance companies directly.
Payments must be made directly from the patient in the form of cash, credit, or electronic transfer.
Many insurance companies will reimburse a portion of your payment.
Dona Wellness Clinic can provide a "superbill" which you may use to submit claims to your insurance company. We do not submit claims on the patient's behalf.
All payment is due at the time of service. A credit card on file with the clinic is required.
Fees for Forms/documentation range from $25-50 depending on complexity if done outside of scheduled appointment times.
Cancellation/No Show Policy
If you are unable to keep your appointment, please provide notice within 2 business days (48 hours), or you will be charged the entire fee of your missed appointment.
